Medical Transcriptionist

Job Description

• Transcribe dictation for a variety of medical reports such as patient histories, physical examinations, emergency room visits, operations, chart reviews, consultation, discharge summaries and/or other reports relevant to area of assignment. Translate medical jargon and abbreviations into their expanded forms to ensure the accuracy of patient and healthcare facility record.

• Take dictation using headsets and transcribing machines; then convert dictated materials or rough noted to written form.

• Distinguish between homonyms, and recognize inconsistencies and mistakes in medical terms, referring to dictionaries, drug references, and other sources on anatomy, physiology, and medicine.

• Review, monitor and edit transcribed reports and Health Information System (HIS) medical diagnostic reports for spelling, grammar, clarity, consistency, and proper medical terminology. Verifies accuracy of patient information such as name, medical record number, location in hospital and patient file location. Identify and monitor mistakes in reports, and check with doctors to obtain the correct information.

• Perform data entry and data retrieval services, providing data for inclusion in medical records and for transmission to physician.

• Return dictated reports in printed or electronic form for physician's review, signature, and corrections, and for inclusion in patients' medical records.

• Answer inquiries concerning the progress of medical cases, within the limits of confidentiality laws.

• Produce statistics from HIS for examiner productivity, statistics for medical diagnostics procedures.

• Produce correspondence and assist medical reports, records, patient-care information, medical research, and administrative material.

• Maintain and adhere to established departmental policies & procedures, objectives, quality improvement programs, safety, environmental and infection control standards.

• Perform a variety of clerical and office tasks, such as handling incoming and outgoing mail, typing, filing, and operating office machines, as might be required.

• Requires sound judgment and deductive reasoning to safeguard the accuracy and integrity of medical dictation, yet must verify the unclear information with responsible physician/ healthcare provider
Mental Demands

• Must have a distinct awareness of the consequence of error in transcribing

• Must be able to pay close attention to detail • Must have the ability to detect medical inconsistencies in dictation
Physical Demands

• Extensive headset use
• Occasional overtime, on call, weekend, holidays work might be required


Diploma Degree in any discipline with Certificate in Medical Transcription, or Bachelor Degree in Healthcare Profession from an accredited institution – Essential
Knowledge of medical terminology science (anatomy, physiology, pharmacology and
investigative procedures) – Essential
Work Experience

Minimum of 2 (two) years’ position-related practical experience – Essential
